By accessing or using the Centre for Rehabilitation and Health Provider Login secure section of this website (the "site") provided by Centre for Rehabilitation and Health Inc., You ("You" or "Your") agree to the following terms and conditions.

These terms and conditions, together with all applicable privacy statement, legal notice and agreements referenced in these terms and conditions, form Centre for Rehabilitation and Health’s agreement with You.

Centre for Rehabilitation and Health may change the site and/or this agreement, including the applicable privacy statement, legal notice and agreements referenced hereto at any time with or without Your consent. Centre for Rehabilitation and Health will notify You of any changes to these terms and conditions upon Your access or use of this site. If You continue to access or use this site after any such change is effective, it is determined that You have accepted the change.

CONFIDENTIALITY, PRIVACY AND SECURITY OBLIGATIONS
  • The information, material and content accessible in the pages of this site, as well as any other information obtained from, or communicated to, Centre for Rehabilitation and Health through the use of this site, including information about an identifiable individual, medical information or any other information which could identify the applicant (or client), spouse and any dependents are strictly confidential (collectively, "information").
  • You will preserve the confidentiality of information and protect and safeguard it against loss or theft, as well as unauthorized access, disclosure, copying, use or modification.
  • You will ensure that all information transmitted or modified through the use of this site is accurate, complete and timely.
  • You will protect and safeguard all information. This includes, but is not limited to, keeping information in a secure area, using passwords to protect files, encryption, protecting information transmitted electronically or communicated by telephone, including information sent by electronic mail.
  • You will keep control of Your username and password. You agree to be liable for all uses of Your username and password, including unauthorized use without Your consent, and You acknowledge that Centre for Rehabilitation and Health shall not be responsible for confirming the identity of any user.
  • You will not communicate, divulge or disclose any information gained by accessing or using this site without the consent of the individual to whom the information pertains.
  • You will promptly advise Centre for Rehabilitation and Health of any complaints or disputes regarding the information.
CONSENT TO COLLECTION AND USE OF USAGE DATA
  • Centre for Rehabilitation and Health will collect certain information about how You interact with and use the site. In connection with this, You agree that Centre for Rehabilitation and Health may collect and use technical data, including but not limited to Your IP address, operating system and browser, to improve Your user experience and the user experiences of others, and facilitate the provision of product support and other services (if any) related to the site. Centre for Rehabilitation and Health may use this information for any purpose described in this paragraph and any related purpose, as long as it is in a form that does not personally identify You.
OBLIGATIONS
  • You will be in good standing with your associated college and this information needs to be uploaded to the software yearly. You will need to provide Proof of Liability insurance yearly. Professional Liability Insurance must be at least $5 million per claim.
  • You will access, use, copy or modify the information contained on this site solely for the purposes of administering the client services and performing the obligations and duties described therein.
  • You will ensure that, if another individual requires access to this site (for billing or administrative purposes only), they will obtain their own username and password.
  • You will upload in PDF format all original records and forms required by Centre for Rehabilitation and Health.
  • A third-party antivirus /malware program must be installed on your devices for extended protection against malware, ransomware and other viruses not captured by Windows Defender.
  • If your computer is infected and you cannot remove / contain the issue with your anti-virus software, get professional help.
  • Keep your operating system and latest version of web-browser up to date.
  • Download any updates for your software or operating system from an official site.
  • You will be required to connect to the software through a secure network. Connecting through public WiFi network is prohibited.
  • You will cease any access or use of the site immediately upon discontinuance of Your duties as a Clinician or termination of employment related thereto.
  • You are to report any breach of privacy to the Centre for Rehabilitation and Health immediately.
  • No information or documents are to be stored on your computer, laptop or any electronic device.
  • For paper records, destruction must be by means of cross-cut shredding, not continuous shredding. This includes and not limited to rough notes, consent forms, reports etc.
  • Client information is transmitted through the secure portal and not via email. If on the rare occasion that email is required, you will not include any client identifiers and you are to reference the CRH case number in the subject line.
VIDEO TELE-PRACTICE
  • Zoom for Healthcare, Ontario Telemedicine Network (OTN) or On Call Health are licensed products that is PHIPA and PIPEDA compliant. There are many other encrypted video platforms that are acceptable for use and authorized by our customers. The platform must be encrypted and meet all Canadian Privacy Laws.
  • CRH requires that CRH clinicians use only encrypted video platforms for remote video sessions that meet Canadian Privacy Laws. These are the only acceptable platforms for video practice.
PROVIDER CANCELLATION POLICY

In light of a trend towards more frequent late cancellations and missed sessions and insurers no longer covering the associated fees, CRH is implementing a new Cancellation Policy moving forward for clinicians. CRH will not charge or collect money directly from the client, however, wish to empower our clinicians to, if they feel it appropriate.

  • CRH maintains that a client will have one grace or missed session where no cancellation fee will be charged, but any missed sessions, or late cancellation (fewer than 24 hours before the appointments) thereafter, the client will be subject to the $100 fee. Only after the consent form and cancellation policy has been reviewed with the client will this apply.
  • The clinicians are responsible for providing the information about the cancellation policy to the client during the introduction call and to be further reviewed when discussing the consent form during the initial session.
  • When a missed appointment or late cancellation occurs, it will be up to the clinician to notify their MCE immediately so the insurer will be advised and asked to remind the client of their obligation to participate. Clinicians must clearly document the time that any cancellation communication from a client was received (date and time) in the software.
  • CRH is encouraging clinicians to handle their no-show fees directly with clients. CRH is suggesting clinicians sign up for "square" so they can bill the client directly and keep the entire cancellation fee. If clinicians handle the fee processing for no shows, receipts must be uploaded to the CRH system, so we have a paper trail of the missed sessions and charges collected for everyone's records.
  • If a clinician chooses not to bill the client directly, clinicians can bill CRH one time for a missed session (after the first grace) at a flat fee of $25.

CRH recognizes that missed appointments and late cancellations have a significant impact on disrupting clinician's schedules and lost income. We truly hope that this will help empower our clinicians to have a system in place where clients will be much more mindful of the consequences of missed appointments, and that overall these will reduce significantly.

GENERAL
  • Failure to comply with any terms or conditions of this agreement may lead to immediate termination of Your access to the site, without limiting any other remedies Centre for Rehabilitation and Health may be entitled to being at law or in equity for further enforcement of this agreement.
  • This agreement will be governed by the laws applicable in the Ontario jurisdiction that governs any applicable client services with Centre for Rehabilitation and Health. In the absence of such client service, or if multiple client services are applicable and are subject to differing jurisdictions, this agreement shall be governed by the laws applicable in the province of Ontario. You irrevocably submit to the jurisdiction of the applicable courts in such jurisdiction. The parties hereby waive any right to a jury trial.
  • You agree that each and every term of this agreement is of the essence. If any one or more of the provisions contained in this agreement should be declared invalid, illegal or unenforceable in any respect, the validity, legality and enforceability of the remaining provisions contained in this agreement shall not in any way be affected or impaired so long as the commercial, economic and legal substance of the transaction contemplated hereby are not affected in any manner materially adverse to any Party. Upon such a declaration, the Centre for Rehabilitation and Health shall modify this agreement so as to carry out the original intent of the Parties as closely as possible in an acceptable manner so that the purposes contemplated hereby are consummated as originally contemplated to the fullest extent possible.
